I understand these messages will be sent by company leadership and will contain only internal operational information, including schedule changes, coverage requests, policy updates, and operational notices.

I understand that no patient information or Protected Health Information (PHI) will be sent via SMS through this program.

I understand that message and data rates may apply based on my mobile carrier plan, and that message frequency varies based on operational need. I understand I may reply HELP at any time for assistance, and may reply STOP at any time to opt out of further messages from this program.

I understand that consent to receive these messages is not a condition of my employment or contractor engagement, and that I may revoke this consent at any time by replying STOP, by notifying my clinic manager or the Chief Operating Officer in writing, or by contacting Human Resources.
